Unspecified Error on NT 4.0 SP 5

I've got a user with NT 4.0 Service Pack 5.  When he tries to run a chart he 

gets an "Unspecified Error" and then a blank screen instead of the chart. 

Has anyone heard of this.  We have cfx4032.ocx, annotatex.dll, and 

sfxbar.dll installed in a common files/software fx shared directory.  All 

have been registered successfully.  Any help would be appreciated.
